There are procedures, established by the law, for filing a claim
against the State and its agencies.
Wyoming Statute § 1-39-113
requires the following:
1. A written claim is to be filed with
the entity:
Department of Administration and Information
General Services Division Risk Management Section 700 W. 21st
Street Cheyenne, Wyoming 82002
2. Not more than two years
after the alleged act, error or omission. 3. The claim shall state:
a. the time, place and circumstances of the alleged loss or injury
including the name of the public employee involved, if known;
b. the name, address and residence of the claimant and his
representative or attorney, if any;
c. the amount of
compensation or other relief requested (with two written estimates on
company invoice or letterhead - if applicable);
d. the claim
shall be signed by the claimant under oath (and notarized) in
substantially the following format:
I,
________________________, have read and understand the provisions of
the false swearing statute. I hereby certify under penalty of false
swearing that the foregoing claim, including all of its attachments,
if any, is true and accurate.
Claims are legal documents and the
claimant may wish to consult his or her own attorney. |
